What is a Medication Review?

A WholeLife pharmacist will look at all the medication that you take, including prescriptions and anything that you buy without a prescription, like vitamins or supplements and review the use and dose to make sure that it’s still appropriate for you. The pharmacist can explain what each medication is for, how it works and when is best to take it. The pharmacist can help you develop an action plan and contact your GP on your behalf if needed.

A medication review most often takes place right in your WholeLife Pharmacy but in some cases, your doctor may ask that the pharmacist visit you at your home to do the review. These are called Home Medication Reviews (HMR) and can only be done by a pharmacist with special accreditation.

Why your should get an In-Pharmacy Medication Review?

There are a few reasons you may need a medication review, including:

  • the medication isn’t having the desired effect, or your condition has deteriorated
  • experiencing symptoms or side-effects
  • significant change to your medications
  • the symptom or condition the medication was treating is no longer relevant
  • changes in your medical condition or new diagnosis
