Prahran WholeLife Pharmacy & Healthfoods is proud to announce that we now have a Diabetes Educator on staff, allowing us to offer dedicated, evidence-based Diabetes Education services to support our patients and community. Managing diabetes can be complex, and every person’s experience is different. Our new service gives you access to specialist guidance, practical education and compassionate support, right here in your local pharmacy.
How Our Diabetes Education Service Supports You
Our Diabetes Educator works closely with each patient to understand their diagnosis, goals and lifestyle, offering tailored education across:
- Understanding diabetes and reducing risk of complications
- Glucose monitoring and making sense of your results
- Medication and insulin support (where applicable)
- Healthy eating and activity guidance
- Building confidence in self-management
- Problem-solving daily challenges
We are committed to empowering you with the skills, knowledge and support you need to manage diabetes with confidence.
Collaborative, Continuous Care
Our Diabetes Educator works alongside your GP and other healthcare providers to ensure your diabetes care is coordinated, consistent and aligned to your broader health needs. Referrals to allied health professionals can also be arranged where helpful.
